Abstract
Pregnancy is a physiological process that a woman experiences throughout the life cycle, however, the level of risk of pregnancy varies based on the risk factors of the mother and fetus. High risk pregnancies are pregnancies that can cause complications for the mother and fetus compared to normal pregnancies. High risk pregnancy based on KSPR is with a pregnancy score of 6-10. The total score is obtained due to the presence of one or more factors experienced by the mother during pregnancy. One of the factors for high risk pregnancy is age ≥35 years and a history of pregnancy interval of 210 years. Pregnant women in the age group ≥35 years are at high risk of complications during pregnancy. Based on the results of a study conducted at PMB Mogita, Tlanakan District, Pamekasan Regency, it was found that there were 35 pregnant women aged 235 years in the register of pregnant women for the 2023-2024 period. ±16 of them had a history of abortion and CS. This Final Project report is case-based with continuous care using Continuity of Care and documented using SOAP management.
Care was provided to Mrs. "I" Age 36 years GIIP1001A000 UK 36 weeks 4 days complaints of frequent urination at night more than 4x, the midwifery care given was to encourage the mother to reduce drinking close to bedtime and reduce the consumption of tea. Pregnancy factors and a history of previous pregnancies make this pregnancy a high-risk pregnancy aged ≥35 years, resulting in greater complications with a KSPR score of 10. Therefore, we recommend that mothers fulfill their needs during pregnancy properly and correctly. At the delivery visit, monitoring is carried out from stage II to stage IV according to the 60-step APN. So Mrs "T" gave birth spontaneously in a healthy condition, with no abnormalities on March 13 2024 at 19:30 WIB at PMB Mogita, Pamekasan Regency, care in the second stage of labor lasted ± 9 minutes, the third stage lasted 5 minutes, the fourth stage lasted 6 O'clock. At the first visit, the two newborns had no complications or signs of infection. At the first and second postpartum visits, breast milk dam complications were discovered on the third day. At the family planning visit, the mother chose the 3-month injectable contraceptive, explained about the 3-month injectable contraceptive, the benefits, side effects, advantages and disadvantages.
From the results of the examination, it was found that the mother's pregnancy did not experience any problems, during the birth process it took place physiologically, during the postpartum period, problems were found with the breast milk dam. After being given complementary therapy, oxytocin massage, there were no problems or complications that occurred for the mother and baby, there was also nothing to worry about. after providing continuous care using Continuity Of Care and Documentation is carried out using SOAP management.
