Neonates are babies who have just experienced the birth process, aged 0-28 days. Newborn babies require physiological adjustments in the form of maturation. Neonates have the highest risk of health problems, one of which is infection which can cause neonatal sepsis. Care was provided to Mrs. U GI P0000 A000 UK 40 weeks at the third trimester pregnancy visit at PMB SETIA on 25 02 2024, a head to toe examination was carried out, documentation was carried out using soap management, she complained of frequent urination. Mrs U PI00I A000 gave birth spontaneously vaginally on 01 03 2024 at PMB SETIA with APN 60 steps which proceeded normally and there was a 2nd degree tear in the birth canal, the baby was born physiologically. Monitoring was carried out 10 times during the postpartum and neonatal period, the results of the baby monitoring process were within normal limits, and the mother had pain from stitching wounds, fear of defecating, sore nipples. followed by family planning care, this care focuses on counseling and informed consent in selecting suitable contraceptives for breastfeeding mothers. Continuity of Care carried out continuous care. Evaluation of the care provided to Mrs. U was overall good. Care is provided based on existing theories in the field. From the care provided, it can be concluded that continuous COC (continuity of care) midwifery care was given to Mrs. U with results, the condition of the mother and baby being healthy. Mrs U can still monitor her condition from pregnancy, childbirth, the postpartum period to following contraception, as well as monitoring her baby's growth and development in health facilities.
