Pregnancy and birth are normal events in life, although they are normal, but the potential for pathology remains. Some of the complications that may occur during a newborn are jaundice. Based on data from the East Java Provincial Health Office in 2021, the number of births was recorded at 51.47% of babies suffering from physiological jaundice out of 420 babies in Indonesia.
The purpose of this final project report is to apply obstetric care to pregnant women in the third trimester, maternity mothers, postpartum mothers, neonates with physiological jaundice and birth control acceptors.
This final project report is case-based with continuous care of care and documented using SOAP management where this care is applied to Mrs. "T" pregnant with her 2nd child aged 24 years who lives in Panglemah Proppo village, Pamekasan Regency.
Mrs. "T's" pregnancy visit was carried out on March 26, 2024 at 07:00 WIB, the mother came to PMB Siti Zainab wanted to check her 2nd pregnancy aged 9 months, not accompanied by complaints. After observation of TTV TD: 120/80 mmhg, Pulse: 82x/min, Temperature: 36.5°C, Respiration: 20x/min, LILA: 23 cm, BMI: 24.5 kg/m², and currently the mother is experiencing chronic energy. In the delivery visit, monitoring was carried out starting from the first period starting on March 27, 2024 at 05:30 WIB with the results of CHPB observation (DJJ: 139x/minute, His: 4x in 10 minutes, duration 45 seconds, PD: 4 cm, Temperature: 36.5°C, TD: 120/80 mmhg). Phase II lasted for 20 minutes, the results of the examination at 09:30 there was an opening of 10 cm, 100% thinning, clear negative amniotic membrane, presentation of the head, denominator of the UUK, Ka, H IV. Phase III lasts for 5 minutes, placenta is born completely at 09:55 WIB, TFU is as high as the center, hard contractions, no lacerations, bleeding ±100 cc, Phase IV lasts for 5 minutes, with an APN of 60 steps, so that Mrs. "T" gave birth spontaneously on March 27, 2024 at 09:45 WIB at PMB Siti Zainab. Childbirth care lasts ± 15 minutes, there is no tear in the birth canal and the baby is born physiologically and there are 3 loose cord twists around the neck, JK: Female, BB: 2800 gr, PB: 48 cm, A/S: 9/10, The care given to newborns at the age of 1 hour includes the administration of eye ointment, vit k 0.5 mg at 10:45, and HB0 0.5 mg at 11:45, as well as daily baby care and breastfeeding. At the first visit, the newborn was found to have physiological jaundice and hypothermy, and at the second visit, the newborn was found to still have physiological jaundice and the umbilical cord was detached. The care provided is to encourage mothers to breastfeed their babies as often as possible on demand and dry their babies in the morning at 07:00-08:00 for approximately 10-15 minutes, try to get indirect sunlight on the baby's skin, therefore the baby should use clothes. The postpartum period was carried out 2 visits with the first visit on 3 days of the postpartum period, and 7 days of the postpartum period Then continued at the visit of Mrs. "T" KB where in this case focused on counseling and inform consent in the selection of contraceptives to be used and the mother chose to use 3-month birth control injections.
Based on the care provided to Mrs. "T", it can be concluded that comprehensive continuous obstetric care is obtained as a result of the mother's chronic lack of energy and good fetus, but in pregnancy it is a physiological pregnancy for TM 3 pregnancy, at the time of delivery the mother does not experience a tear of the birth canal. During the postpartum period, mothers are advised to make routine visits at least once a week and during family planning it is recommended to use birth control that does not inhibit milk production, so that mothers can minimize the occurrence of complications both in pregnancy and family planning.
