Abstract

Newborns really need nutritional intake of breast milk (ASI) for newborns. If the baby's nutrition is not met, it will cause metabolic disorders in the baby, one of which is an increase in bilirubin, if the bilirubin level increases it will cause jaundice (yellowing). Physiological jaundice will appear 24-72 hours after birth and will disappear within 2 weeks. This Final Assignment Report is case-based with continuous care in a Continuity of Care manner and documented using SOAP management where this care is applied to "newborn babies." with physiological jaundice AT PMB MOGITA Pamekasan Regency "At the third trimester pregnancy visit, comprehensive care is provided starting from data collection to evaluation and documentation using SOAP management. At the pregnancy visit, Mrs "S" found a problem with frequent urination. Her care advised the mother to drink more mineral water during the day and limit drinking at night so that it would not disturb the quality of the mother's sleep. At the delivery visit, a referral was made because her amniotic fluid had run out. And Mrs "S" gave birth via Sectio Caesarea (SC) on March 26 2024 at 12:05 WIB at RSU Moh Noer. At the first visit the baby lost weight and experienced physiological jaundice. wake the baby if it has been asleep for more than 2 hours, tell the mother to breastfeed until the breast feels empty. At the second visit the baby no longer had jaundice. During the postpartum period, it was found that Mrs "S" was afraid when defecating because the surgical scar was afraid of coming off. Her care gave support to the mother not to worry that the stitches would not come off and did not feel pain when defecating and encouraged the mother to Defecating in a sitting position and still not bathing because she is afraid of getting scars from the operation. Her parents told the mother to maintain personal hygiene, namely by wiping her body with water and using the correct toilet method, and changing the sanitary napkin every time it was full. And the second postpartum visit was not found. a problem and a danger sign that leads to complications. Then it was continued with a family planning visit for Mrs "S" which focused on counseling and informed consent in choosing the contraceptive to be used and the mother chose to use 3-month injectable birth control.
