Abstract

Pregnancy and childbirth is a condition that all women will experience, both newly married and those who have been pregnant and gave birth. Primigravida with height < 150 cm and LILA < 26 cm is one of the factors in the occurrence of high-risk pregnancies that can cause complications or adverse effects on the mother and fetus, both problems from the mother and fetus. Therefore, continuity of care must be carried out in order to detect early complications or problems suffered by the mother during her pregnancy, as has been applied to Mrs "M" GI P000 A000 UK 39 single weeks of intrauterine life the impression of a normal birth canal is /u good mother and fetus

On the first visit on the 3rd day the baby looks yellow because the baby is not breastfed and breast milk is not smooth, on the 2nd visit the 7th day the baby is normal and no longer yellow, during the puerperium on the first visit on day 3 it is found that breast milk is dammed, because breast milk is not being breastfed in her baby, and on the 2nd to 7th day visit there were no problems and danger signs that lead to complications. Then continued on the KB visit Mrs. "M" which focused on counseling and informed consent in the selection of contraceptives to be used and the mother chose to use a 3-month contraceptive injection.

From the results of the examination, it was found that the mother's pregnancy had no problems with maternal TB <150 cm and LILA <26 cm with a KSPR score of 4, in the delivery process there were no complications or problems, during the puerperium there were complications or problems that occurred in the mother, namely breast milk dam in the mother. On the third day the baby was also found to have icterus on day 3 and on day 7 the baby was not icterus. The method of ibi's final project report (LTA) is case-based with continuous midwifery care or continuity of care and the approach uses SOAP.

Based on the care given to Mrs. M it can be concluded that continuous midwifery care is based on Continuity Of Care for Mrs "M" with the result that the condition of the mother and fetus is good but the mother's pregnancy is normal, so mothers are advised to increase awareness in carrying out routine checks starting from pregnancy to family planning to midwives / personnel health so that it can be detected early
