Pregnancy and birth are events that are awaited by every woman in life, although pregnancy here is a special situation, it does not rule out pathological events that can cause pregnancy complications. Some of the complications of pregnancy is anemia. Efforts to prevent this from happening by implementing continuous/comprehensive care starting from pregnancy, childbirth, postpartum, newborn and family planning, this final project report (LTA) method is case-based with continuous midwifery care based on continuity of care, and its approach using SOAP.

This care was carried out on Mrs. “E” GIIPl00lA000, UK 39 weeks, alive, single, head position, intrauterine, normal walking impression, general condition of the mother with chronic lack of energy and good fetus. At the first visit of the third trimester of pregnancy, comprehensive care was carried out from data collection to evaluation and documentation using SOAP management. During the pregnancy visit, Mrs. "E" found problems, namely back pain and groin pain. From the results of the pregnancy examination, the mother was examined for TFU after being calculated using the TBJ formula (28-12) x 155 = 2.480 grams) it was found that she would give birth to a baby with low birth weight. (LBW). At the delivery visit, monitoring was carried out starting from the first stage to the fourth stage with an APN of 60 steps, so that Mrs "E" gave birth spontaneously on April 9, 2021 at 05:30 WIB at PMB FLAMBOYAN.

Delivery care lasted ± 20 minutes with bleeding caused by a second degree birth canal tear and the baby was born at 06:05 WIB spontaneously without complications and abnormalities with low birth weight (LBW). On the 7th day visit the baby had physiological jaundice on the 7th day of the postpartum mother there was no problem and on the 23rd day the baby and the postpartum mother did not find any problems and danger signs that could lead to complications. Then after 6 weeks postpartum, it was continued on the KB visit Mrs. "E" where this visit focused on counseling and informed consent in the selection of contraceptives to be used. Mrs “E” chose to use a 3-month birth control injection on the grounds that it is very affordable in terms of price and does not interfere with the breastfeeding process.
